The City of Chilton is accepting applications to add names to their veteran’s memorial.
The memorial is located at Linkner Park and honors the men and women who served in all the US Military branches.
Residents are asked to submit a DD214 and Chilton Veterans’ Memorial Form to the city hall located on School Street.
Form DD214 can be obtained at the Veterans Service Office at the Calumet County Courthouse.
It costs $325 for a four-by-eight name plate that provides space for three to thirteen characters or spaces in each line.
An eight-by-eight plate costs $550 and includes space for six lines with thirteen characters or spaces in each line.
Applications only run until March 1st.
Names are added to the Veterans’ Memorial once a year.
