Aurora Medical Center – Manitowoc County has earned recognition from the Institute for Healthcare Improvement.
The hospital received an Age-Friendly Health System- Committed to Care Excellence for its excellence in care for older adults.
Age-Friendly Health System is an initiative through the John A. Hartford Foundation and the Institute for Healthcare Improvements in partnership with the American Hospital Association.
The award is given to hospitals, practices, convenient care clinics, and nursing homes for their work toward reliable practice of a set of four-evidence based elements of high-quality care, known as the 4Ms (What Matters, Medication, Mentation and Mobility).
Aurora Manitowoc County credits itself for its care for older adult’s specific health outcome goals, use of age-friendly medications, mobility or mentation, and identifying and treating and managing dementia and depression.
President Cathie Kocourek says she’s proud of her team’s commitment to constantly improve care delivery to all patients and ensure age-friendly care for the community.
